Black is almost a no-no when dealing with realism and portraiture. I have done many portraits without the use of it entirely. In this portrait, with the black clothing, it is necessary.
Black is used only on the dress, earrings, a touch in the deepest corners of the mouth, iris and pupil of the eye, and the eyelashes. Limited use of the darkest color in this way gives an amazing depth to the eyes.
LOOK AT THOSE TEETH! YEEUUCH!
The teeth have a lot of color on them, partly overspray, partly intentional spraying. Teeth are almost NEVER white, unless reflecting a lot of light. Remember the need for contrast with the white of the collar? The same principle applies here.
